Diabetes Prevention

We fostered outreach and awareness to expand access to the National Diabetes Prevention Program, and
to reduce the incidence of type 2 diabetes in our community. 

To increase awareness about prediabetes we:

• Worked with KUCB, the local radio station, to participate in the Tuesday Checkup Series. Our involvement with these shows is to pair Tribal members and staff with medical providers to speak to a wide range of topics that holistically encourage wellness. • Shared information about prediabetes, prevention, and NDPPs in the Tribe’s quarterly newsletter which is mailed to 85 households. Three generations gather to discuss diabetes prevention.

To support prediabetes screening, testing, and referral to CDC-recognized type 2 diabetes prevention programs we’ve:

• Collaborated with the tribal clinic to offer A1C testing and at a local health fair.

• Collaborated with the community clinic to help their team develop and send a letter from a trusted physician to 255 patients sharing information about prediabetes, the CDC risk test, and the opportunity for free enrollment in InquisitHealth and Omada’s NDPP programs.

To expand the reach of CDC-recognized type 2 diabetes prevention programs we’ve:

• Widely advertised the NDPP and helped three community members enroll in InquisitHealth or Omada

To develop culturally-relevant approaches to increase program participation and retention among AI/AN members we’ve:

• Trained three Tribe staff as diabetes prevention life coaches through the Prevent T2 curriculum. However, because of the COVID-19 pandemic we have decided not to offer in-person classes at this time. Instead we sought out, and have focused on promoting, virtual programs like InquisitHealth and Omada.

Inquisithealth is a phone based diabetes prevention program.

Services include talking with a life coach once a week, and overviewing lessons together. The durationg of this program is 1 year.
